How to get your Alumni Card

Sign up for the Alumni Club and agree to registration of your personal information in the Alumni Club database.

Digitally:  By completing the digital form Application for a UP Alumni Card, including uploading a photograph (either JPEG or PNG format, 35 x 45 mm) and being acquainted with the conditions of the card’s use and the provisions in the form Application for a UP Alumni Card. The form is only accessible to registered alumni.

The alumna/alumnus will be informed by e-mail when the card is ready. The Alumni Card will be send to your postal address (given in personal settings).

What the Alumni Card does

The UP Alumni Card exclusively allows UP alumni to make use of certain university benefits, including benefits at external partners of UP.

The UP Alumni Card offers card holders a wide range of benefits at UP, especially lending privileges at the library, accommodation at the UP Accommodation and Dining Facilities, and preferential rentals of UP rooms, buildings, and grounds. At the same time the UP Alumni Card allows cardholders a slew of benefits in the areas of culture, entertainment, sports, tourism, and other services available in the City of Olomouc.

The alumna/alumnus on the basis of filling out the form Application for a UP Alumni Card below (hereinafter, “The Form”) will obtain, under the conditions below, an identification card with the UP logo, embedded with a contactless electronic chip. The Card will list the cardholder’s name(s), surname(s), identification number, the cardholder’s photograph, and their relationship to UP.

Fees

The first 1500 UP Alumni Cards will be issued at no charge.

Rules

The UP Alumni Card can only be obtained by the alumna/alumnus who has properly filled in the required personal details with successful subsequent authentication of the information in the UP Alumni database. After the authentication of the alumna/alumnus has been completed, forms delivered until 15th of each month will be processed. The card will be send at the end of month to your postal address (given in personal settings).

The alumna/alumnus who has attended studies at multiple faculties is allowed to complete the Application for a UP Alumni Card only once, and that for the last field studied.

The UP Alumni Card can also be issued to a UP employee who is a UP alumna/alumnus; however the card does in no way replace or stand in for their UP Employee ID Card.

The UP Alumni Card has no expiration date.

By submitting form, I am aware and agree that Univerzita Palackého v Olomouci, IČ: 61989592, Křížkovského 511/8, 771 47 Olomouc (Palacký University Olomouc) can have access to and make use of my personal data, namely: Name(s), Surname(s), Titles, Personal Identification Number, Date of Birth, E-Mail Address, Telephone Number, Permanent Address, and Photo, for the purposes of creating the UP Alumni Card and including me into the UP Alumni database, and with no time limitations.

By submitting form I attest that I am also fully aware and consent that my personal information listed above will be gathered and processed by UP employees.

Furthermore, by submitting form I attest that I am also fully aware and consent that my personal information listed above has been given voluntarily. If I decide not to provide the above-listed information, or any specific item of information listed above, I understand that it will not be possible for me to register into the UP Alumni database, nor receive a UP Alumni Card.

Furthermore, by submitting form I attest that I am also fully aware and consent that according to the provisions stated in Section § 12, of Law No. 101/2000, Czech Law Coll., on Personal Data Protection, and subsequent amendments, on the right to information on the processing of one’s personal and sensitive information (i.e. the right to access all information about one’s person) and according to the provisions stated in Section § 21, Paragraph 1, of Law No. 101/2000, Czech Law Coll. on Personal Data Protection, and subsequent amendments, I have the right to request the administrator or processor for an explanation, if I feel that the administrator or processor has used my personal information in a way which is inconsistent with the protection of my personal and private life, or in violation of the law, especially regarding misuse of my personal data. Furthermore, I hereby acknowledge that I have the right to request the administrator to correct the faulty state of the information, particularly by means of blocking access to, correcting, filling in, or deleting my personal data.

Furthermore, I hereby acknowledge by submitting form that all rights and responsibilities in processing personal data are to be carried out according to the established legal statute of Law No. 101/2000, Czech Law Coll., on Personal Data Protection, and subsequent amendments, and related laws and subsequent amendments, unless a special law states otherwise.

And by submitting form I agree to the system of borrowing library documents from the UP Library without further proof of identification and I affirm that the electronic register of its circulation system is legally binding. I affirm that before signing this application I have been acquainted with the UP Library Rules and the operational rules of individual sections of the UP Library available at http://www.knihovna.upol.cz; and when using these services, I will follow all rules and regulations, especially those pertaining to protecting library documents from damage, ruin, or loss; I will respect the loan period for the borrowed library documents; and in the case of damage, ruin, or loss of library documents or not returning items by their due date, I accept the penalties associated with them, especially the payment of contractual penalties and fees in the manner stated in the UP Library Rules. By signing this application, I further state that I will use copies of these documents in accordance to established laws for educational and study purposes, and solely for my own use and for non-commercial purposes, and that I will abide by the ethics of their citation according to pertinent legal stipulations and the norms of ČSN – Czech Technical Standards.
